Divorce Final - One Parent Has Passed Away - Need to obtain sole cusotdy

My ex-husband and I are divorced.  We have joint custody of our two children aged 11 and 16.  We reside in Ontario.  The children have resided with him since our separation.  He passed away on August 12, 2012.  The children are obviously with me at the moment.  They want to live with me.  What forms do I need to fill out and take to the Court House in order to obtain sole cusotdy?  I have looked at a number of forms from the Ministry Website, but they seem to be geared more towards a custody battle (i.e. FLR-08 and FLR-35-8).  There is no custody battle.  He has died, the children wish to reside with me and I need sole custody in order to register them for school, apply for passports, etc.  I know that I will need a Certified Copy of the Death Certificate, the Divorce papers and the Separation Agreement to take to the Court House, but what else do I need to fill out?  Thanks, S
